Why BMI Doesn't Use Age or Gender (and What Does)

A common assumption when searching for a BMI calculator is that a good one should account for age and gender, the way many other health metrics do. The standard adult BMI formula deliberately doesn’t — and understanding why clarifies both what BMI is actually useful for, and which other metrics to use when age or gender genuinely matter.
The formula, exactly as designed
BMI = weight (kg) ÷ height (m)2

BMI was developed in the 1830s by Adolphe Quetelet as a population-level statistical tool, not an individual diagnostic measure — its original purpose was describing how body weight scaled with height across large groups of people, not evaluating any one person’s health. That population-statistics origin is part of why age and gender were never built into the formula: it was never designed to be individually diagnostic in the first place.
The CDC explicitly notes that for adults, BMI is interpreted using the same fixed weight-status categories regardless of age or sex — but for children and teens, the interpretation method is completely different, using age- and sex-specific percentiles instead of fixed categories.
Why this becomes a real limitation for individuals
Muscle mass tends to decline with age even at a stable weight, meaning an older adult and a younger adult with the identical BMI can have meaningfully different body fat percentages.
Women's bodies typically carry a higher essential body fat percentage than men's at an equivalent BMI, a physiological difference the formula doesn't capture.
Two 30-year-olds, one male and one female, with the identical BMI can have noticeably different body fat percentages, purely because of typical sex-based differences in body composition — a gap the formula has no mechanism to account for, since it never looks past height and weight.
